Job Description:

Exact Staff has immediate opportunities for Care Coordinators for a well known healthcare facility. Care Coordinators are tasked with the responsibility of identifying and assisting members that display a variety of social and/or emotional needs and make arrangements with these member's primary care provider in an effort to provide better medical management.

Job Duties

  • Engage with members in the Care program
  • Monitor treatment and staying up to date with provider recommendations
  • Provide health promotion and self management training
  • Assist members and/or families regarding discharge issues and transition of care needs
  • Participate in medical appointments as necessary
  • Document interactions with members and providers as needed
  • Independently assess the psychosocial functioning needs of patients and formulate a treatment plan
  • Home visits if needed

Job Requirements

  • Excellent written, verbal and interpersonal communication skills
  • Strong leadership skills
  • Ability to work independently
  • Ability and willingness to treat all patients with the utmost kindness and consideration
  • Must be a graduate of an accredited school of nursing with a current CA LVN license or Certified MA
  • Two years in care coordinating
  • Bilingual preferred

Compensation

$16.00 - $19.00Per Hour
