Job Description:
Exact Staff is currently seeking a skilled Case Manager for our client in Arvin, CA. Must have a strong background in social servicesand behavioral management. Under the supervision of management staff, the Case Manager will provide supportive services to individuals and low-income families, helping them access resources, and match to adequate resources. Schedule: Monday–Friday, 8:00 AM–5:00 PM
Pay Rate: $20.51/HR

Essential Duties and Responsibilities:
- Provide case management services to clients with social service and behavioral needs.
- Conduct intake interviews and assessments to identify client needs, goals, and match to resource programs
- Develop individualized service plans and monitor progress toward goals.
- Connect clients with community resources, housing options, and supportive programs.
- Provide behavioral management guidance and support to clients and families.
- Maintain accurate and timely records, reports, and case documentation.
- Collaborate with outside agencies, service providers, and internal teams to ensure effective support.
- Conduct home visits, outreach, and follow-ups as needed.
Qualifications:
- High school diploma or GED required, Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred.
- At least 2 years of experience in social services, behavioral health, or case management.
- Strong interpersonal communication skills with the ability to work effectively with diverse populations.
- Ability to interpret and apply policies and procedures while maintaining confidentiality.
- Computer literacy and strong organizational skills.
- Bilingual (English/ Spanish) is required
Physical Demands:
- Must be able to communicate effectively in person and by phone.
- Must be able to operate a motor vehicle and travel locally for home visits and client support.
- Ability to occasionally lift/move up to 35 pounds.
