What You’ll Be Doing:
· Build and maintain collaborative relationships with patients, families, healthcare providers, and community stakeholders to improve patient care and health outcomes.
· Assess, identify, and prioritize patient and community health education needs to support preventive care and wellness initiatives.
· Assist in the planning, implementation, and program completion goals while maintaining budget guidelines and ensuring compliance
· Provide individual and group health education on topics including chronic disease management, HIV/AIDS, asthma, cardiac health, diabetes, family planning, obesity, pregnancy, smoking cessation, and disease prevention.
· Conduct patient assessments, orientations, and provide education, referrals, care coordination, and assistance with program enrollment and required documentation for specialized programs, including Comprehensive Perinatal Services Program (CPSP), Family PACT, and Every Woman Counts.
· Participate in patient population management as it relates to clinical services while taking into account cultural diversity and local resources.
· Coordinate post-hospital discharge follow-up by contacting patients, scheduling recommended follow-up appointments, and collecting information necessary to ensure continuity of care.
· Support through self-management by educating high-risk patients while recognizing situations requiring referral to a Registered Nurse for clinical intervention.
· Serve as a patient advocate by collaborating with interdisciplinary care teams, including providers, registered nurses, licensed clinical social workers, medical assistants, and health educators to coordinate comprehensive patient care.
· Participate in quality improvement initiatives, including data collection, chart audits, performance improvement activities, and evidence-based disease management programs.
· Maintain age-appropriate health education and support to patients across the lifespan, including infants, children, adolescents, adults, and geriatric populations.
· Maintain knowledge of mandatory reporting requirements and established protocols related to domestic violence, child abuse, and dependent adult abuse.
· Provide feedback in the development and modification of health education protocols.
· Coordinate health education materials, including audio-visual materials for presentations.
· Maintain approved health education materials at assigned sites.
· Work closely with medical assistants and nursing staff to integrate health education into health center operations and patient visits.
· Attend and actively participate in department meetings, program meetings, staff meetings, and other assigned activities.
· Prepare and maintain written documentation of health education program activities as required by Omni Family Health policies and state and federal regulations.
· Collect data, surveys, evaluations, and documentation of activities to meet reporting requirements in accordance with contractual agreements, policies, and procedures.
· Participate in community groups, coalitions, health fairs, and other outreach activities on behalf of Omni Family Health.
· Perform additional duties and special projects as assigned by management.
Qualifications:
· Associate's Degree with at least two (2) years of experience in similar work or counseling work in a community outreach program.
· Other medical training, experience, or certifications may be considered on a case-by-case basis.
· Bachelor's Degree in a related field preferred.
Certifications
· Current and valid Basic Life Support (BLS) certification.
· CPSP certification or ability to obtain certification within one year of employment.
· Willingness to obtain certification in Health Education and/or Family Planning.
· Willingness to obtain education, training, or experience with chronic diseases.

· Ability to work effectively with individuals of diverse ethnic, religious, and economic backgrounds.
· Willingness to work at different locations and weekends as needed.
Pay, Schedule, and Location
- Pay Rate: $22/HR
- Schedule: Full-Time
- Shifts Available: Monday-Friday 8AM-5PM
